Bronze struck medal with obverse depicting nude male bust facing right with legend: "JACOBUS BERZELIUS./NAT. MDCCLXXIX. DEN. MDCCCXLVIII." Reverse depicts standing winged youth leaning with his left hand onto pedestal with attributes of chemistry, pouring liquid with his right hand from a flask into bowl held by female with serpent, seated at left; legend above: "NATURAM JUSSIT VIRES PROFERRE LATENTES"; inscription in exergue: "FUNDATORUM SUPREMO LUGENS/MEDIC.SVEC.SOCI. 'A'." Medal is inset into square black leather fitted case with dark red silk velvet interior.
