Bronze struck plaquette with obverse showing facing bust-length portrait of Robert Fulton within circular panel at upper left labelled: "ROBERT FULTON"; mountain range and river navigated by steamship labelled: "CLERMONT" in background; banner with inscription below: "1890 FULTON TRUST COMPANY/OF NEW YORK/1915." Reverse bears inscription: "COMMEMORATING/THE TWENTY-FIFTH ANNIVERSARY/OF THE/FULTON TRUST COMPANY/OF NEW YORK/1890-1915." Medal is set into rectangular white cardboard box.
