Bronze struck medal with obverse showing front left-facing bust-length portrait of Perry. Reverse shows tower at water front; legend: "PERRY'S VICTORY CENTENNIAL/ 1813 1913/ OFFICIAL MEDAL". Slot hanger fused to top; medal hangs by red, white, and blue silk ribbon from brass pinback labeled "INTER-STATE COMMISSIONERS".
