White metal struck medal with obverse showing clothed busts of Lincoln, at left, and Johnson, at right, in oval medallions inscribed "LINCOLN" and "JOHNSON", facing each other, with eagle above holding scroll inscribed: "FREEDOM TO ALL MEN"; shield below at center is inscribed: "WAR/FOR THE/UNION." Reverse bears inscription within central roundel: "FOR PRESIDENT/ABRAHAM LINCOLN/OF ILLINOIS/FOR VICE PRESIDENT/ANDREW JOHNSON/OF TENNESSEE"; legend: "REPUBLICAN CANDIDATES 1864."
